UPDATED Form for Students Attending Colleges or Universities Requiring Covid-19 Injections

10-5-22 Updated Form. Disregard below and CLICK HERE for form.

The first version of this form was written in May by Corey’s Digs for The Solari Report, and has been updated this week by the Solari team to account for future events, as things evolve. Please review this important information, legal and medical resources, and form to submit to colleges or universities.

[NOTE TO STUDENT: Be sure to document the date and time you submit the form to your school; also document the date and time and their response if they refuse to sign it. Note that three other Solari Report forms1,2,3 are also available as downloadable PDFs: the “Family Financial Disclosure Form for Covid-19 Injections”; “Notice and Declaration of Parental Authority Requirement of Disclosure and Safety of Medical Treatment/s”; and “Form for Employees Whose Employers Are Requiring Covid-19 Injections under Emergency Use Authorization.”]

The situation with Covid-19 injections is fluid and evolving rapidly. As of July 2021, three vaccines were being administered in the U.S. under Food and Drug Administration (FDA) emergency use authorization (EUA): the Pfizer-BioNTech and Moderna mRNA injections and the Janssen/Johnson & Johnson adenovirus-vectored injection.

The injections’ regulatory status could soon change. On July 16, the FDA granted priority review to Pfizer’s application for full approval (licensure) of its Covid-19 vaccine for those age 16 and up. Moderna and Johnson & Johnson are also preparing to seek full approval. FDA officials have pledged to issue a decision with respect to the Pfizer application within two months. And though “not intervening in the decision of government scientists,” President Biden predicts full approval by September or October.4

Approval could have significant ramifications for college students. Assuming one or more shots receives full approval, public health experts expect “a wave of Covid-19 vaccine mandates,”5 confidently anticipating that licensure will remove legal and public relations barriers, including for colleges and universities that want to—or are being pressured to—require vaccinations for their students. Even under EUA, over 600 colleges have already decided to require student or employee injections for fall 2021.6,7,8

American students who do not wish to accept Covid-19 injections are thus in a difficult position. The genesis of this form was to provide a tool for students to use within the context of emergency use authorization with schools that are attempting to mandate still-investigational Covid-19 injections (Scenario 1). With the possibility of full FDA approval in fall 2021 (Scenario 2), the legal context for U.S. mandates will likely shift in significant ways. In the event of licensure, we will provide an updated form that reflects the altered regulatory circumstances.

Under both scenarios, we anticipate that a form of this type will remain a critically important tool for students determined to educate their college or university about the full set of physical and financial risks they are asking students to incur. Students should strongly consider reviewing the form with school officials and also sharing it with other students. This could result in a positive outcome for students and, if used widely, could be a game-changer.

Scenario 1: Emergency Use Authorization

The number of colleges and universities trying to deny students’ right to choose and their right to bodily integrity—by requiring EUA Covid-19 vaccines as a condition of enrollment or on-campus residence—is growing. Under EUA, however, such requirements are a violation of the law. That said, on July 26 (after the Department of Veterans Affairs mandated the injections for its health care personnel), the Department of Justice published an opinion stating that it is legal for “public and private entities” to mandate Covid vaccines despite the injections only being approved for emergency use.9 We anticipate that this should and will be challenged.

Mandating the EUA injections is also a violation of fundamental human and religious rights and medical ethics.10 As the Association of American Physicians and Surgeons has stated, “Students and their families may justifiably believe these policies discriminate against individuals who aren’t candidates for this vaccine, have pre-existing conditions, previous COVID-19 disease, cite religious objections, or are otherwise exercising their freewill choosing not to participate in this optional vaccine experiment.”11 Lawsuits against university mandates are already wending their way through federal courts.12,13

The provisions of the 2005 Public Readiness and Emergency Preparedness (PREP) Act and a February 2020 declaration by the U.S. Department of Health and Human Services (HHS) Secretary mean that pharmaceutical companies cannot be held liable for injuries or deaths caused by EUA Covid-19 injections.14 However, other companies, institutions, and individuals can be held liable.

On the grounds that the injections are under EUA and are not formally licensed, many health care providers are refusing service to individuals injured by the Covid-19 injections, and health and life insurance companies are declining coverage for vaccine-related injuries and deaths.15 This denial of care and coverage poses a serious threat to the physical and financial well-being of students presented with a requirement to get a Covid injection—perhaps even more so than the possibility of losing their place at school for not being vaccinated. Medical bills for vaccinated individuals with serious injuries have already exceeded $1 million in some cases.16 Under the PREP act, the vaccine-injured may submit a claim to the Countermeasures Injury Compensation Program (CICP), but the CICP historically has rejected 92% of claims and has not yet compensated a single Covid injection claim.17

Reviewing this form with colleges/universities may open educational institutions’ eyes to the regulations in place and the liability they may face (that they may otherwise not be aware of). If a student should decide to accept the required experimental Covid-19 injection, the form also provides a mechanism to insist on a signature from the school’s representative that holds the college or university liable. If the school’s representative refuses to sign the form, that may be an indicator of their concerns about potential liability and should prompt an immediate discussion about how the student can attend school without being injected.

Scenario 2: Full Approval

Mandating that college students get some licensed vaccines is not new, although all but five states allow religious and medical exemptions and some permit philosophical exemptions.18 Many states require incoming students to get licensed vaccines against hepatitis B and meningitis, as well as pushing other “catch-up” or recommended vaccines (such as influenza, HPV, Tdap and MMR).19

Under a scenario of full FDA approval for Covid-19 injections, an even broader cross-section of colleges and universities will likely feel emboldened or pressured to impose school mandates. Evoking the specter of “potentially severe” Covid illness, some commentators are suggesting that the process for granting medical or religious exemptions “may look a little different” (i.e., be more restrictive).20

If Covid-19 injections lose their EUA status and receive full FDA approval, they would no longer enjoy the liability protection conferred by the PREP Act. However, if the FDA extends full approval to children under age 18—as is widely anticipated21—and the Centers for Disease Control and Prevention (CDC) recommends Covid-19 injections for “routine administration” to children (or pregnant women),22 manufacturers and health care providers will be protected from liability for Covid-vaccine-related injuries and deaths under the 1986 National Childhood Vaccine Injury Act (NCVIA).23 From that point on, the only avenue available for compensation would be the notoriously stingy and difficult-to-navigate National Vaccine Injury Compensation Program (NVICP).24

Legal Resources

If you find yourself in a predicament with a college or university that refuses to hear you out, does not provide you with required information, or threatens you with expulsion, here are a few potential resources for attorneys:

Medical/Scientific Resources

Comprehensive, high-integrity medical and scientific information, including briefs and reports summarizing what is known about the risks of Covid-19 injections and adverse events, is available (and routinely updated) at the following websites:

  • America’s Frontline Doctors: In addition to its legal resources (see above), AFLDS’ website includes information about Covid-19 treatments, “issue briefs” on post-vaccination complications and other topics, and numerous other resources.
  • Children’s Health Defense (CHD): In addition to reporting on vaccine safety topics via its flagship online news outlet The Defender, CHD offers a “Fighting COVID Mandates” toolkit and related resources, a series of eBooks (including eBooks on vaccine mandates and protection of individual rights), a research library, and a “Community Corner.”
  • Doctors for COVID Ethics (D4CE): Among the excellent resources provided by this group of doctors and scientists from 30 countries are “Letter to physicians: Four new scientific discoveries regarding the safety and efficacy of COVID-19 vaccines,” which warns that “all physicians must reconsider the ethical issues surrounding COVID-19 vaccination,” and a 23-page Expert Statement outlining the serious risks of the Pfizer injection (European brand name Comirnaty) in children.

Form for Students Attending Colleges or Universities Requiring Covid-19 Injections under Emergency Use Authorization

NOTE TO COLLEGE OR UNIVERSITY: As a student at your institution, I am requesting that you review this document, provide the requisite information, and sign the form, in regards to your requirement that students get a Covid-19 emergency use authorization (EUA) experimental vaccine.

1) If I agree to receive an EUA Covid-19 injection, does my student health insurance plan [if applicable] provide complete coverage should I experience an adverse event, or even death?

2) As a student at your institution, will you provide any other medical or financial support to me or my family if I have an adverse event to an EUA Covid-19 injection and am unable to attend school for days, weeks, or months, or if I am disabled for life?

3) The Food and Drug Administration (FDA) requires that EUA vaccine recipients be provided with certain vaccine-specific information to help them make an informed decision about vaccination.1 The EUA fact sheets that must be provided are specific to each authorized Covid-19 injection and are developed by the manufacturers of the injections (Pfizer/BioNTech, Moderna, Oxford/AstraZeneca, and the Johnson & Johnson subsidiary Janssen). The fact sheets must provide up-to-date information on the injections and their ingredients; vaccine recipients must also receive information about adverse events. Have you read, understood, and provided me (and all other students) with these fact sheets and current information on adverse events—and can you furnish a list of vaccine ingredients guaranteed to be complete—so that I/we can make an educated decision?

4) Have you reviewed the material adverse events experienced to date by people who have received EUA Covid-19 injections, reported to the Vaccine Adverse Event Reporting System (VAERS)?2 Reported adverse events include death, anaphylaxis, blood clots and related complications, heart problems (myocarditis and pericarditis), neurological disorders, autoimmune disorders, other chronic and inflammatory conditions, blindness and deafness, infertility, fetal damage, miscarriage, and stillbirth.

5) The FDA’s guidance3 on emergency use authorization of medical products requires the FDA to “ensure that recipients are informed to the extent practicable given the applicable circumstances…[t]hat they have the option to accept or refuse the EUA product…” Are you aware of this statement? Have you informed all students that they have the option to refuse?

6) With respect to the emergency use of an unapproved product, the Federal Food, Drug and Cosmetic Act, Title 21 U.S.C. 360bbb-3(e)(1)(A)(ii)(I-III) reiterates that individuals be informed of “the option to accept or refuse administration of the product, [and] of the consequences, if any, of refusing administration of the product, and of the alternatives to the product that are available and of their benefits and risks.”4 In the event that the FDA decided to grant full approval to Covid-19 vaccines, state legislation would be required to allow companies to mandate the Covid-19 injections. Are you aware of these facts?

7) EUA products are unapproved, unlicensed, and experimental. Under the Nuremberg Code—the foundation of ethical medicine—no one may be coerced to participate in a medical experiment. The individual’s consent is absolutely essential. No court has ever upheld a mandate for an EUA vaccine. In Doe #1 v. Rumsfeld, 297 F. Supp. 2d 119 (2003),5 a federal court held that the U.S. military could not mandate EUA vaccines for soldiers: “[T]he United States cannot demand that members of the armed forces also serve as guinea pigs for experimental drugs” (Id. at 135). Are you aware of this?

8) The United States Code of Federal Regulations6 and the FDA require the informed consent of human subjects for medical research. The EUA Covid-19 injections are unapproved, unlicensed, investigational vaccines that are still in their experimental stage. It is unlawful to conduct medical research on a human being, even in the event of an emergency, unless steps are taken to secure the full informed consent of all participants. Are you aware of this?

9) According to Federal Trade Commission (FTC) Guidelines7 and the FTC’s “Truth In Advertising,”8 promotional materials—and especially materials involving health-related products—cannot mislead consumers, omit important information, or express claims. All of this falls under the rubric of “deceptive advertising” (whereby a company is providing or endorsing a product), whether presented in the form of an ad, on a website, through email, on a poster, or in the mail. For example, statements such as “all students are required to get the Covid-19 vaccine to make the campus safe” or “it’s safe and effective” leave out critical information. Critical information includes the facts that Covid-19 injections are unapproved EUA vaccines that “may” or “may not” prevent Covid, won’t necessarily make campuses safer, and could in fact cause harm. Not providing links or attachments of the manufacturers’ fact sheets and current information on adverse events is omitting safety information. Are you aware of this?

10) Since the Covid lockdowns began over one year ago, there have been over 178 reported breaches of unsecured protected health information (PHI), incidents investigated by the Office for Civil Rights (OCR). These breaches exposed millions of people’s personal health information. Although many of these incidents were attributed to hacking, some of the breaches to PHI fell directly under the 1996 Health Insurance Portability and Accountability Act (HIPAA), such as sharing a patient’s or person’s information with an unauthorized individual or incorrectly handling PHI.9 Can you please explain your obligations to me, under HIPAA law, with respect to your requirement that I receive this injection?

11) Whereas pharmaceutical companies that manufacture EUA vaccines have been protected from liability related to injuries or deaths caused by experimental agents since the Public Readiness and Emergency Preparedness (PREP) Act10 was enacted in 2005, companies and all other institutions or individuals who mandate experimental vaccines on any human being are not protected from liability. Are you aware that you do not enjoy such liability protection?

12) Are you aware that students or families could file a civil suit against you should the student suffer an adverse event, death, or expulsion from school for refusing an EUA Covid-19 injection?

As the legally authorized officer of the college or university, I have read all of the above information, have provided my students with all of the information that the FDA requires be provided to recipients of the Covid-19 injections, and do hereby agree to assume 100% financial responsibility for covering any and all expenses from adverse events, including death, through insurance coverage or directly. In addition, I affirm that the student shall not be denied access to school should they decline to receive a Covid-19 injection.

Absolutely excellent Corey. Thank you for gathering details (devils in the details) & legal references.
I called Primary Health here in Garden City, Idaho. Nurse was totally unaware shots are experimental even though she administers them to patients. She just, “hands out the paperwork”. I asked if paperwork said experimental & she said she did not know.
As a semi-retired RN I am shocked & see it as unethical that a provider administering shots would not have educated their employees or that nurse would have not research this unapproved these “concoctions”-(sorry but it was what dame to mind) described as a vaccine.
Thank you again Corey for your diligence. Love & appreciate the thoroughness you put into your digs!! May God Bless you & yours.

  • Susan L Hough

Thank you Corey!!! I have been looking for info for both my college student and high school student on this and appreciate all you do to keep us informed and armed with the truth. This is a hard one on parents. God Bless you!

  • Robyn

It goes against the Florida compilation of school discipline laws and regulations part V 1003.573
Its called PROHIBITED RESTRAINT period done over

  • David Robertson

Oddly enough, the word concoction also came to my mind recently when describing the contents of this Jab. It is a witches’ brew of ingredients apparently designed to enslave us forever. Thanks, but no thanks.

  • Cecille Chan

I think we are at a time when the Cabal puppets do not care what information you show them or what papers you have them sign as long as their primary aim of genocide is carried out. The Constitution, the precedents from the Nuremberg trials, and the God-given natural human rights be damned. This Deep State/NWO totalitarian society is barreling into our reality at “warp-speed” greatly aided by the complacency of the populace while waiting for “the plan” to be carried out. We were actually horses being led to slaughter, not water/freedom. Here are the people you have been looking up to, to preserve your freedom. They all belong to the same “club” George Carlin often spoke about. “And you are not in it”. It is long, but you will see your “hero” towards the end of the video. I’ve done my share. Over three decades of trying to wake up people to the reality of the NWO. Most of the time, all I got in response were scoffs. I am worn and weary. Even my last name feels like a curse right now. The useful idiots are zeroing in on that.

  • Hanna

Thank you so much. It is exhausting to be in the minority regarding this subject. I’m stunned there are so many willing to put their life in jeopardy and are sucked into this advertising campaign. We have been very upset as parents of teenagers, one who has plans to attend a university that requires this shot.
Cecille, I think of George Carlin often now, I can picture him saying “See!! I told you!!”.

  • Frank Furter

It’s nice to see an Asian-American who knows what is really going on! Thank you for your comments. I too, get very frustrated at seeing so many people walking around thinking “they (the government) are here to take care of us.” Sometimes I feel like I’m walking amongst a sea of zombies. Is it the fluoride in the water? How did people get so dumb? One contributing factor I think is that most people have no solid knowledge of good and evil and think that “everyone has good intentions.” Keep up your good work, and please don’t lose hope! We (those who know the truth) need to stay strong and stick together!

  • Mary

Thank you Corey Digs. We remain strong if we all stay strong and united. I believe we are too many if we stay strong and stay firm. Freedom is our call!

  • alice

What a thorough and excellent piece of work. Thank you.

I don’t think most Brits realise that the vaccinations aren’t approved. We are a nation of guinea (?) pigs. I don’t know anyone else who has declined to be vaccinated and people are puzzled that I won’t have it. The Lord designed me with an immune system so why would I need their witchcraft? Not listening to main stream media makes all the difference – this country was pounded with rhetoric, an endless stream of lies and … it seems to have worked. Next step, passports – bring ’em on.

  • Kim

Is there a form for nursing home or assisted living residents if they are told they must get the vaccine??Thank you

  • Leonie

Here’s a very good video of 5 very important specialist doctors discussing the “vaccine” or bioweapon as they call it. Please pass it on to everyone- Farcebook will delete it so I went through Messenger but if you’re on another platform go for it. Copy and paste below.

  • Rochelle eisele

Thank you for this information. I’m with all of you. I have three teenagers and I’ve been up many nights thinking , what will I do when they are told you are forced to get this vaccine or no college , no sports . We all need to fight this. Where is our rights? This was the land of the free – how can they force us to put something in our kids bodies ? This has to be stopped.

My friend’s grandson took the jab as required for college entrance. He took an antibiotic afterwards for an illness. He’s now at a burn center with blisters all over his body. Who knows what the lasting effects will be?

  • Susan Anderson

Thank you for this information. Do you have something similar for work related vaccine requirements? Susan

This is so informative with links to every possible scenario and further request for proof and credible sites and links with documentation as evidence. Well structured and to the point. I am in South Africa and they are rolling out the vaccines at this moment which both my parents have gone for. I have since been investigating this medical experiment. My folks took the first jab of the Pfizer vaccine. People in South Africa are either ignorant or don’t want to challenge the status quo. I on the other hand am a single parent of a teenage boy and if they touch my son with a vaccine I am gong to lose it.
I have therefore decided to challenge the status quo myself (single mom alone in Cape Town), by going to the schools, the big and small corporates, the vaccine clinics are my biggest target centers (nurses doctors and patients in the waiting rooms and those leaving). I am also going to our media houses as I would like to know why the heck is none of this being spoken about in South Africa? Especially seeing as how the first trials were held here and the African people refused as they know what Bill Gates and colonial countries did in our country ie. using people in Africa as guinea pigs, leaving women infertile, test test test!

People must be made aware of what is happening and they can call me whatever they want, if I save even one persons life that’s ok with me. If I make at least a few people aware and rethink what they are doing then I am glad. My parents had the first jab with no information given to them either verbally or on paper, no warning of possible side effects, no paper telling them what to do if they experience those side effects, no after care info pamphlet or sheet, no advice on how or even where to report adverse reactions ( and I have checked-we have our own version of VAERS as well). What does that mean? It means no one will be reporting anything wrt adverse reactions and no one will be documenting anything!
No one is making anyone else aware of the other side of this thing! Utterly shocking!
People waiting for someone to fight for them or speak up? I don’t know.

I have therefore further decided to setup a system (be it by email and manually adding it to a database) myself. I lost my a job thanks to covid lockdown. I am taking the little bit of cash I do have to print informational leaflets and also doing some video clips interviewing and questioning the people at the clinics as well as the staff at these clinics. If they know this is illegal (and I will be making lists to record those citizens who continue to administer this vaccine after my information is given to them. Mainly at the clinics to post links via a website that I am currently setting up.
The fact that people are not being monitored and not being recorded and definitely not being advised on any aspect of any part of this mass death vaccination makes me so angry!

Thanks so much for this information it really helps my 1 citizen cause to make people aware and awake in South Africa. Our media reports nothing of this illegal testing on human subjects. I want to know why!

  • Merlyn J. Travis

    Corey, this is excellent.

    1. Do you have a any data on people using it …succesfully? I have one colleague that just got totally ignored.

    2. We are developing a powerful coalition for “informed consent” on campuses and wish to integrate your work here.

    3. Would like to do some collaborative investigative work with you on who is pushing the college mandates.

I should note that question 12 is written in such a way to indicate that the school will be held responsible if the student suffers death/adverse reaction if they REFUSE the vaccine, which leads me to believe that no one with signature authority at a school/university would be willing to sign this.

Nearly every institution allows for religious/philosophical exceptions, so I think it would likely be more prudent to ask the school or agency if this is allowed than try to get them to sign this, as this feels much more antagonistic than informative.

  • Nicole Macera

Hi i see you have a form to send to collages, My son’s collage isn’t mandating this shot, but he plays football and the NCAA Liberty conference in PA is , they are blackmailing (coercing) our student athlete into getting this shot even if they already had Covid-19 they will not allow that to be a exemption even though you should have natural immunity. Is anyone else on here in the the same position we don’t know what to do. thank you – Nicole M.

  • Denise Wesley

Has anyone had any success with this? My daughter, just yesterday, was notified by her college that students living on campus and attending classes are required to be vaccinated or they will have to commute. Strange that the commuters, faculty and staff do not have to be vaccinated though. If no success with the form, does anyone have a document/letter that they’ve used to seek either religious or medical exemption from the vaccine? I’m desperate. I don’t want my daughter’s college year to be negatively impacted due to this. I’m so sad for her. Thank you so much.

  • Shelly

Hi Corey, can fed, state or private employers require weekly COVID testing if you decline the vaccine? This is going to start happening in California.

  • April Grass

Corey you do such amazing work and I am so grateful for you! My 18 year old daughter is about to attend Purdue University in Indiana. What Mitch is doing isn’t much better than IU, yes we still get a choice but in the beginning he was doing some segregation and many parents were “yay”. We are living in an upside down world, where people don’t even see evil. We don’t have enough parents at Purdue fighting the “conservative” Mitch. (sarcasm) The only good thing is that with the delta variant, all will be wearing a mask (Protect Purdue) now so she will not be targeted in that respect. here’s the link to their utter bs.
I heard you tell the Edge a deep dive into the National Sexual education would be a good dive.
I also think another good dive, the mental health of our children the last twenty years would be another.
They keep saving how much they care about the children! We know what they want!
Thank you again for all you do!!!
    Many Blessings,

  • Pam

Now that the FDA has approved the pfizer vaccine, a lot of the points in the student form are invalid. Can you make another form available by tomorrow as the deadline is September 1st. Or can I just modify the form by taking out the word “experimental” or taking out some points that don’t apply anymore?

Thank you so much for your hard work gathering this information for us. Is there an updated form for Students Attending Colleges or Universities Requiring Covid-19 Injections under FDA Approval? I am currently a student attending medical school and I am being forced to take the covid-19 vaccine or else face dismissal from my school. It is sad how this government is taking away our freedom. This vaccine will do more harm than good. This government only intends to control us but I refuse to succumb to their or any other entities’ threats.
